Job summary

Open Sky Community Services is seeking a licensed clinician or a candidate pursuing licensure to provide assessment, treatment planning, and therapeutic support across residential and community-based settings. The Clinician conducts initial assessments, develops individualized plans, and applies person-centered, evidence-based practices to empower individuals toward independence.

Qualifications

• Master’s degree in Social Services or related field required.
• Licensed Practitioner of the Healing Arts or licensed eligible with timelines for LCSW, LADCI, or LMHC.
• Valid driver’s license with acceptable driving record.

Responsibilities

• Collaborate with care coordination partners, healthcare providers, and community agencies to ensure integrated support.
• Develop and maintain partnerships with community-based organizations to strengthen the agency’s network.
• Conduct individual counseling sessions, facilitate support groups, and lead treatment planning meetings.
• Provide clinical consultation, guidance, and training to the interdisciplinary team.
• Participate in on-call rotation and critical incident review, including potential on-site response.
