Job summary

Generations Family Health Center is seeking a Clinical Therapist (LCSW/LMFT/LPC/LMSW/LMFTA/LPCA) in Willimantic, CT. The Behavioral Health Department offers full- or part-time positions with a competitive benefits package.

Qualifications

• Master’s degree in social work or related field.
• Licensed candidates preferred (LCSW/LMFT/LPC/LMSW/LMFTA/LPCA).
• Experience with Substance Use Treatment is preferred but not required.
• Experience with patients across the lifespan, including under 18, preferred.
• Bilingual candidates (Spanish/English) preferred based on patient population.

Responsibilities

• Conduct individual, group, family and/or couples therapy sessions for patients.
• Develop and coordinates a clinical treatment program for the diagnosis or referral of patients with behavioral health needs.
• Provide multidisciplinary approach to treatment plan, coordinating with medical providers and other clinicians.
• Assess patient progress and modify treatment plan accordingly.
• Coordinate care of patient with referring physician or agencies.
• Document all therapies according to established protocols.
• Attend required meetings and participate in committees as requested.
• Participate in professional development and maintain continuing education hours for licensure.
• Maintain patient confidentiality and document case records on all contacts.
