Qualifications

• MSW or MA in Counseling or related field required.
• 0–3 years of social work experience.
• Under supervision by LCSW or LPC with approved supervision.

Responsibilities

• Perform psychosocial assessments and develop treatment plans.
• Provide psychotherapeutic counseling and education to patients.
• Complete all documentation per policy.
• Adhere to licensure regulations and standards.
• Assess Social Determinants of Health and refer as needed.
• Collaborate with patient and family for discharge planning.
• Reassess and modify treatment plans as needed.
• Maintain patient information per legal and policy standards.
• Respond to emergencies with the medical team.
• Tailor care to patient population needs.
