Qualifications

• Master's degree in Social Work or Counseling or related field.
• 0-3 years of total work experience in Social Work or related fields.
• Supervision by LCSW or LPC with approved supervision or ACS credential.

Responsibilities

• Provide psychosocial assessments and develop treatment plans.
• Deliver psychotherapy or counseling (individual and/or group) under supervision.
• Document all services per policy and regulations.
• Identify patient needs and provide case management and referrals as needed.
• Coordinate with families and care teams for discharge planning.
