Qualifications

• Master's degree in Social Work (MSW), Master's degree in Counseling, or related fields.
• 0-3 years of total work experience in Social Work or related fields.
• Work under supervision of a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC).
• Ability to perform psychosocial assessments and develop treatment plans.
• Knowledge of crisis intervention theory and its application.

Responsibilities

• Provides comprehensive psychosocial assessments and develops treatment plans.
• Delivers behavioral/medical health treatment and education (individual and/or group).
• Completes documentation per department and hospital policy.
• Maintains patient health information per regulations and policies.
• Collaborates with patient, family, and care team to discharge planning.
