Curant Health is seeking a PMP Pharmacist to deliver personalized medication therapy management and case reviews via telephone. The role focuses on optimizing outcomes through collaboration with prescribers and healthcare teams.

You will educate patients on medication adherence, monitor progress, and document activities across clinical software platforms, ensuring HIPAA compliance and high-quality care.

Qualifications

• Two years of experience in a retail or mail-order pharmacy preferred.
• HIPAA knowledge required.
• 340B knowledge is a plus.
• Must obtain dual licensure as a registered pharmacist in Georgia and Florida.
• AAHIV Certification preferred.

Responsibilities

• Provides medication-related clinical services to patients via telephone.
• Reviews records to determine the appropriateness of medication therapy.
• Evaluates patient conditions to ensure issues are treated.
• Follows up with PMP enrolled patients and providers to meet drug treatment goals.
• Consults with prescribers to resolve medication-related issues.
• Maintains documentation of medication activities in software platforms.
• Provides education and training on medication use and adherence.
