Job summary

JobRx, Inc. is seeking a Clinical Pharmacist to optimize medication management and support physicians in patient care. You will collaborate with care teams to ensure appropriate therapies, provide adherence coaching, and coordinate transitions of care.

The role involves telephonic follow-up, workflow-driven medication management, and documentation in Engage systems to support patient outcomes and care continuity.

Qualifications

• PharmD degree required.
• Active TX state pharmacy license or market license.
• Residential state license in good standing.
• Clinically required licenses in good standing.
• PIC required licenses in good standing.

Responsibilities

• The Clinical Pharmacist reports to the management staff. Daily tasks will also be directed by the pharmacy services team.
• Provides telephonic follow up with members to provide medication reconciliation and medication management to assigned patients including transitional care and chronic disease support.
• Complete assigned tasks related to medication management services.
• Work with member, physician, health system(s), pharmacy services, and other care providers to meet the member's individual needs based on protocols and referrals.
• Coordinating transitional management including obtaining medication orders and completing medication reconciliation.
• Work with Engage team to ensure medication provided until PCP follow-up.
• Provides medication management services utilizing prescribed workflows and completes appointments in accordance with workflow.
