Curant Health Georgia LLC is seeking a PMP Pharmacist to provide individualized patient management and medication therapy review by phone in Smyrna, GA. The role focuses on optimizing therapeutic outcomes through outbound and inbound communications with prescribers and patients.

The position requires at least two years in a retail or mail-order pharmacy, HIPAA knowledge, and dual licensure in Georgia and Florida; 340B is a plus and AAHIV certification is preferred.

Qualifications

• Minimum of two years of experience in a retail or mail-order pharmacy environment preferred.
• Knowledge of HIPAA/Patient Confidentiality guidelines required.
• Must obtain dual licensure as a registered pharmacist in Georgia and Florida.
• AAHIV Certification preferred.

Responsabilités

• Provides medication-related clinical services to patients via telephone.
• Reviews records of patients to determine the appropriateness of medication therapy.
• Evaluates patient’s condition to ensure all issues are being treated.
• Uses motivational interviewing to improve adherence to medication.
• Follows up with PMP enrolled patients and providers to meet drug treatment goals.
• Consults with prescribers to resolve medication-related issues.
• Maintains documentation of medication-related activities in software platforms.
• Provides education on proper use of medications and adherence.
• Provides follow-up and monitoring with patients and prescribers.
