Concierge Home Care is seeking a Clinical Team Support (Scheduling Coordinator) in Bradenton, FL to ensure efficient scheduling for clinicians and timely patient care. This in-office role supports daily operations with strong organization and communication, using WellSky (Kinnser) EMR.

The position offers Monday–Friday, 8:30 AM–5:00 PM, with a compensation of $18–$20 per hour and opportunities for growth within a growing Florida-based home health provider.

Qualifications

• High school diploma required.

Responsibilities

• Schedule patient visits per physician orders and care plans.
• Coordinate clinician schedules to optimize productivity and coverage.
• Manage daily scheduling changes, call-outs and urgent visits.
• Answer and triage incoming calls to support operations.
• Track medical supplies and support RAP/EOE processes.
• Maintain accurate scheduling and documentation in the EMR.
• Provide administrative support to the clinical team.

Your Role as Clinical Team Support

In this role, you will support daily clinical operations by coordinating schedules and ensuring seamless communication between patients and clinicians.

Responsibilities include:

• Scheduling patient visits in accordance with physician orders and care plans
• Coordinating clinician schedules to optimize productivity and coverage
• Managing daily scheduling changes, call-outs, and urgent visit needs
• Answering and triaging incoming calls to support clinical operations
• Tracking medical supplies and supporting RAP/EOE processes
• Maintaining accurate scheduling and documentation within the EMR
• Supporting the clinical team with administrative tasks as needed

Schedule: Monday-Friday, 8:30 AM - 5:00 PM (in-office role)

Compensation: $18-$20 per hour
