Job summary

Concierge Home Care in Jacksonville, FL is seeking a Clinical Manager (RN) to lead a dedicated home health team, ensure Medicare compliance, and uphold clinical excellence.

You will supervise clinicians, verify documentation, coordinate with physicians, and drive quality patient outcomes while supporting growth of the organization. This office-based role runs 8:30-5:00, Monday–Friday with bonus opportunities and strong benefits.

Qualifications

• Active Florida RN license is required.
• Minimum 1 year RN experience (acute care, med/surg, ICU, ER, or similar).
• At least 1 year of skilled home health with OASIS proficiency.
• Strong knowledge of home health regulations and clinical compliance.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Supervise and support home health clinical staff.
• Review documentation to ensure Medicare and regulatory compliance.
• Monitor quality of care and patient outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Coach clinicians to support professional development and clinical excellence.

Your Role as Clinical Manager (RN)

8:30- 5:00, Monday–Friday, leadership role (primarily office-based)
