Job summary

Vitra Health in Boston, MA is seeking a Clinical Support Registered Nurse to provide home and community-based clinical support, focusing on care coordination, patient education, and chronic disease management, while ensuring MassHealth and insurer documentation requirements are met.

As part of the Healthcare Delivery team, you will collaborate with interdisciplinary staff, conduct visits, education, and care planning, and document accurately in EMR while traveling to client homes as needed.

Qualifications

• RN license in Massachusetts (required).
• Minimum of 2 years of recent experience with older adults or individuals with disabilities in a clinical, home health, case management, or related setting.
• Strong communication, interpersonal, and client engagement skills.
• Experience with care coordination, patient education, or case management preferred.
• Proficiency with electronic medical records and basic technology tools.
• Valid driver’s license and reliable transportation for travel to client homes.

Responsibilities

• Supports scheduled client visits with caregiver education, monitors client status, and assesses changes.
• Supports semi-annual care plan reviews, updating goals and interventions with nursing teams.
• Provides education on health maintenance, preventative care, chronic disease management, and medication adherence.
• Supports clients and caregivers in navigating healthcare systems and accessing community resources.
• Participates in multidisciplinary care planning, case reviews, and discharge planning.
• Maintains accurate, timely, and compliant documentation per regulatory standards.
• Communicates professionally with clients, caregivers, and healthcare partners in person and remotely.
• Ensures all client visits are scheduled, conducted, and documented per MassHealth regulations.
