Job summary

Vitra Health is seeking Registered Nurses to support community‑based care across Massachusetts. You will conduct intake, health assessments, and home visits, while educating clients and caregivers and coordinating with healthcare providers. Travel within the region is required, with a focus on delivering compassionate, patient‑centered services.

This role emphasizes care coordination, documentation quality, and multidisciplinary collaboration to help clients remain safe and empowered at home.

Qualifications

• Massachusetts RN license required and current.
• Minimum 2 years direct care experience with elders or adults with disabilities.
• Willingness to travel within assigned region.
• Fluency in English; second language preferred.
• Strong communication, documentation and organizational skills.
• Solution oriented with ability to multitask.

Responsibilities

• Complete intake and annual assessment and admission visits.
• Complete documentation including client assessment forms and multidisciplinary plan of care; communicate with PCP and other providers.
• Educate clients and caregivers; provide non-clinical support.
• Perform regular in-home visits and caregiver education/skills training.
• Conduct semi-annual visits; update plans of care and review medications.
• Travel to client homes and community settings; coordinate care with team.
