Job summary

Vitra Health is seeking a Community Support Specialist in Massachusetts to build relationships with providers and community agencies, coordinate social services, and evaluate client needs. The role involves planning care with nurses, maintaining documentation, and delivering culturally appropriate health education to clients and caregivers.

The position requires 2–3 days at the Braintree HQ and 2 days in the field, with travel to client homes and community settings.

Qualifications

• One year of clinical experience with elderly or disabled populations.
• Knowledge of medical terminology.
• Proficient in Microsoft Office and Google Suite.
• Bilingual in English and a second language preferred.
• Valid driver's license with insurance.

Responsibilities

• Conduct on-site visits with clients and caregivers.
• Obtain social history and perform psycho-social evaluations.
• Develop and review AFC/GAFC care plans with nurses and case managers.
• Train, supervise AFC caregivers with the RN and case manager.
• Document care-management progress notes after visits.
• Assist participants in finding community, financial, and legal resources.
• Coordinate social services with the care team and discharge plans.
• Support clients with ADL/IADL tasks and provide orientation to programs.
