Qualifications

• At least one year of clinical experience working with elderly or disabled populations.
• Demonstrated knowledge of medical terminology.
• Advanced proficiency in Microsoft Office Suite and Google Suite (G Suite).
• Valid driver's license with proof of insurance required.
• Bilingual in English and secondary language preferred.
• High School Diploma Required.

Responsabilités

• Develop and maintain positive relationships with providers, healthcare organizations, and community service agencies to ensure comprehensive support for all program participants.
• Coordinate the delivery of social services, evaluate clients, and formulate individualized plans of care with nurses and healthcare providers.
• Maintain neat and organized client charts and ensure documentation complies with company policies and state regulations.
• Provide culturally appropriate health education, information, support, and counseling to clients and caregivers.
