Job summary
Vitra Health in Massachusetts is seeking a licensed practical nurse (LPN) to provide nonclinical support, educate clients and caregivers in health maintenance and prevention, and assist in navigating healthcare services.
You will travel to client homes and work with a multidisciplinary team to ensure high-quality, person-centered care for older adults and adults with disabilities, while promoting independence and safety.
Qualifications
- Two years of recent experience in direct care of elders or adults with disabilities.
- Massachusetts nursing license is required.
- Valid driver's license with proof of insurance.
- Bilingual in English and a secondary language preferred.
- Experience in healthcare, home health care, case management, or sales preferred.
Responsibilities
- Complete intake and annual assessments with nursing.
- Complete monthly in-home visits and caregiver education; assess changes.
- Conduct semi-annual visits and update care plans with nursing.
- Assist with obtaining information and accessing other services.
- Participate in multidisciplinary care planning and discharge planning.
- Communicate professionally with clients, PCPs and providers.
