Job summary

Vitra Health is seeking an LPN Case Manager in Massachusetts to educate clients and caregivers, navigate healthcare options, and support in-home care coordination. This role emphasizes advocacy for clients to remain safely at home and deliver person-centered care with a multidisciplinary team.

Responsibilities include intake, annual assessments, in-home visits, plan-of-care development, and collaboration with nursing staff, PCPs, and community resources.

Qualifications

• Licensed by the Massachusetts Board of Registration in Nursing.
• Two years of recent direct care experience with elders or adults with disabilities.
• Excellent interpersonal and communication skills required.
• Valid driver’s license with proof of insurance.

Responsibilities

• Perform intake, annual assessments, and admission visits with nursing.
• Conduct monthly in-home visits and caregiver education; assess psychosocial and environmental changes.
• Review and update plans of care; collaborate with nursing and caregivers.
