Eventus WholeHealth is seeking a Clinician Support Specialist (CSS) to back the clinical care team in skilled nursing and assisted living settings. You will coordinate workflows, support clinical services, and facilitate communication among patients, providers, and care teams.

The CSS role emphasizes clinical support, administrative coordination, and customer service with strong organizational focus in a fast-paced healthcare environment.

Qualifications

• High school diploma or equivalent required.
• CMA/CNA/PCT certification or eligibility preferred.
• Strong customer service and communication skills.
• Ability to manage multiple priorities in a fast-paced clinical environment.

Responsibilities

• Prepare for daily clinical rounds with complete patient information and schedules.
• Assist clinicians with encounter documentation, data entry, and chart preparation.
• Coordinate and maintain accurate patient schedules, including admissions, discharges, and acute changes.
• Serve as liaison between clinicians, facility staff, patients, and families to support communication and care quality.
• Assist with triage needs and documentation as directed by the clinician.
• Organize and manage clinical documentation, including lab results and patient records.
• Host and support telehealth visits for routine and acute care needs.
• Obtain and process patient consents and route documentation properly.
• Participate in facility meetings and relay updates to the care team.
• Support discharge planning and ensure accurate census and records.
