Eventus WholeHealth is transforming care in skilled nursing and assisted living facilities through an integrated, evidence-based model. The Clinician Support Specialist (CSS) plays a key role supporting the clinical team and coordinating workflows to deliver high-quality patient care.

You will work closely with Advanced Practice Clinicians and facility staff to enhance communication among patients, providers, and care teams.

Qualifications

• High school diploma or equivalent.
• CMA/CNA/PCT certification preferred.
• At least 1 year of patient care experience.
• Valid driver’s license and reliable transportation.
• Excellent verbal, written, and electronic communication.
• Strong organizational skills and ability to manage multiple priorities.

Responsibilities

• Prepare for daily clinical rounds with patient information and schedules ready.
• Assist with encounter documentation, data entry, and chart preparation.
• Coordinate patient schedules including admissions, discharges, and changes.
• Serve as liaison between clinicians, facility staff, patients, and families.
• Assist with triage needs and documentation as directed by the clinician.
• Organize and manage clinical documentation, including lab results and patient records.
• Host and support telehealth visits for routine and acute care needs.
• Obtain and process patient consents and ensure proper routing of documentation.
• Participate in facility meetings and communicate updates to the care team.
• Support discharge planning and ensure accurate census and records.
