Job summary

Eventus WholeHealth is transforming care in skilled nursing and assisted living facilities through an integrated model. The Clinician Support Specialist (CSS) coordinates workflows with Advanced Practice Clinicians and facility staff to ensure efficient, high‑quality patient care and clear communication among patients, providers, and teams.

Qualifications

• High school diploma or equivalent.
• Certified/Registered Medical Assistant, CNA, or PCT preferred.
• One+ year patient care experience preferred; medical scribing a plus.
• Valid driver's license and reliable transportation.
• Strong customer service and professional communication skills.
• Excellent written, verbal, and electronic communication; highly organized.

Responsibilities

• Prepare for daily clinical rounds with patient information and schedules.
• Assist clinicians with encounter documentation, data entry, and chart prep.
• Coordinate patient schedules including admissions and discharges.
• Serve as liaison between clinicians, facility staff, patients and families.
• Assist with triage and documentation as directed by the clinician.
• Organize clinical documentation including labs and diagnostics.
• Host and support telehealth visits for routine and acute care needs.
• Obtain and route patient consents and ensure proper documentation flow.
• Participate in facility meetings and communicate updates to the care team.
• Support discharge planning and maintain accurate census and records.
• Maintain professionalism and positive representation of the organization.
