Job summary

Eventus WholeHealth is seeking a Clinical Support Specialist to bolster the clinical care team in skilled nursing and assisted living facilities. You will coordinate workflows, support clinicians, and enhance communication between patients, providers, and care teams.

Your role combines clinical support, administrative coordination, and customer service in a fast-paced healthcare environment. Regular facility travel is required, with a focus on quality patient care and care coordination.

Qualifications

• High school diploma or equivalent is required.
• CMA/CNA/PCT certification or registration is highly preferred.
• Minimum one year of patient care experience is preferred (medical scribing experience also preferred).
• Valid driver’s license and reliable transportation are required.
• Strong customer service, communication, and organizational skills are essential.

Responsibilities

• Prepare for daily clinical rounds by ensuring patient information, documentation, and schedules are ready.
• Assist clinicians with encounter documentation, data entry, and chart preparation.
• Coordinate and maintain patient schedules, admissions, discharges, and changes.
• Act as liaison between clinicians, facility staff, patients, and families to support care delivery.
• Assist with triage and documentation as directed by clinicians.
• Organize and manage clinical documentation, including lab results and records.
• Host and support telehealth visits for routine and acute care needs.
• Obtain and route patient consents appropriately.
• Participate in facility meetings and communicate updates to the care team.
• Support discharge planning and maintain census accuracy.
