Qualifications

• Must possess a Master of Social Work (MSW) degree from an accredited college or university.
• Valid and unrestricted license to practice social work (LCSW or LICSW).
• Must possess a valid drivers’ license.
• Must attest to no disqualifiers per Driver Safety Policy.
• Must have reliable transportation.
• At least five years of experience in a social work capacity, preferably in a medical setting, with supervisory experience.

Responsibilities

• Participates in Interdisciplinary Team’s initial assessments, care planning and on-going re-evaluations.
• Provides clinical services to participants and families in psychosocial assessment and treatment.
• Provides on-going case management for participants as needs and status changes.
• Coordinates resources for participants and families and refers to appropriate community/government agencies.
• Supervises and directs the Social Work Department and conducts performance appraisals.
• Keeps Site Director informed of employee relations issues and other matters as needed.
