Responsibilities

• Delivers comprehensive pharmaceutical care to patients
• Reviews medication orders for accuracy, appropriateness and efficacy
• Discusses medication order clarifications with prescriber, updates orders to reflect changes, and notifies involved personnel
• Documents all pharmacy interventions in patient EHR and CDS system
• Monitors drug therapy for appropriateness of use, dose, regimen, and interactions
• Provides pharmacokinetic consultations and dosing recommendations as required
• Prevents, detects and reports adverse drug reactions and medication errors
• Participates in drug therapy management of medical emergencies (code blue)
• Performs admission history and discharge medication review and counseling to facilitate transitions
• Provides operational support to Clinical Pharmacists and Specialists as needed
• Responds to drug information requests and optimizes pharmacotherapy regimens using current literature
• Promotes rational, cost-effective drug therapy
• Participates in formulary management and enforces non-formulary procedures
• Assists in procurement of medication and verifies home medication where applicable
• Facilitates implementation of clinical and operational initiatives
• Recommends and evaluates drug inventory par levels
• Documents clinical consultations and cost avoidance in the clinical documentation program
• Develops formulary monographs or MUE as assigned by leadership
• Mentors pharmacy technicians, students, and residents; provides supervision as needed
• Precepts IPPE and APPE rotations and PGY1/PGY2 residents as appropriate
• Participates in scholarly activities including research, publications, and posters
