Job summary

The role emphasizes pharmacokinetic dosing, drug information, adverse event monitoring, formulary management, mentorship of technicians and students, and collaboration with physicians to optimize patient outcomes.

Qualifications

• Education: ACPE-accredited College of Pharmacy
• Post-Graduate Year 1 (PGY-1) Pharmacy Residency preferred
• Licensed or license eligible as a pharmacist in Kentucky (required)
• Current board certification preferred
• Basic Life Support (within 90 days) and ACLS as determined by leadership (required)

Responsibilities

• Delivers comprehensive pharmaceutical care to patients
• Reviews medication orders for accuracy and efficacy
• Documents all pharmacy interventions in EMR and CDSS
• Monitors drug therapy for appropriateness, dose, regimen, and interactions
• Provides pharmacokinetic dosing consultations and other recommendations
• Mentors pharmacy technicians, students, and residents
