Job summary

UofL Health in Louisville, KY is seeking a Clinical Staff Pharmacist to deliver comprehensive pharmaceutical care across neonatal to geriatric patients in a teaching hospital setting.

Responsibilities include medication order evaluation, drug distribution, sterile and extemporaneous compounding, formulary management, and staff mentorship, collaborating closely with physicians, nurses, and other health professionals.

Qualifications

• Graduated from an ACPE accredited College of Pharmacy (required)
• Post-Graduate Year 1 (PGY-1) Pharmacy Residency or three (3) years of relevant work experience (preferred)
• Licensed or license eligible as a pharmacist in the Commonwealth of Kentucky (required)
• Current board certification by Board of Pharmacy Specialties® (preferred)
• Current certification in Basic Life Support or must obtain within 90 days from date of hire (required)
• Current certification in Advanced Cardiac Life Support (ACLS) as determined by pharmacy leadership (required)

Responsibilities

• Delivers comprehensive pharmaceutical care to patients
• Reviews medication orders for accuracy, appropriateness and efficacy
• Discusses medication order clarifications with prescriber and updates orders accordingly
• Documents all pharmacy interventions in patient EMR and CDS systems
• Monitors drug therapy and provides pharmacokinetic consultations as needed
• Prevents and reports adverse drug reactions and medication errors
• Mentors pharmacy technicians and students when applicable
