Job summary

Baptist Health is hiring a Clinical Staff Pharmacist for the 3rd shift to provide safe, effective, and timely medication therapy across centralized and decentralized systems. Responsibilities include drug information, clinical services, antimicrobial and opioid stewardship, and patient counseling.

Graduate Pharmacists with KY licensure or PharmD with hospital experience are preferred; BLS/ACLS may be required.

Qualifications

Graduate of an accredited college or university with Pharmacy degree.
Licensed Pharmacist with the Kentucky State Board of Pharmacy in good standing.
Pharm.D. or relevant clinical hospital pharmacy experience is desirable.
Must provide documentation of state licensure and CE as required by the state of KY.
Advanced clinical training or PGY1 residency preferred.

Responsibilities

Provides comprehensive decentralized or pharmacy clinical services including anticoagulation management/monitoring, pharmacokinetic dosing, renal dosing, and nutritional support services.
Participates in antimicrobial stewardship and opioid stewardship.
Provides medication counseling to patients.
Works with inter-professional patient care teams to help patients with transitions to care, including discharge.
Serves as a drug information expert and resource regarding medication management.
