Baptist Health in Kentucky is seeking a Clinical Staff Pharmacist for the 3rd shift, full-time. You will provide safe, effective medication therapy, participate in centralized and decentralized systems, and support antimicrobial and opioid stewardship while ensuring high-quality patient care.

The role requires KY licensure, Pharm.D., and BLS/ACLS certifications, with advanced clinical training preferred. You will work with inter-professional teams on patient care units and contribute to

Formación

• Graduate of an accredited college or university with Pharmacy degree.
• Licensed Pharmacist with the Kentucky State Board of Pharmacy in good standing.
• Pharm.D. or relevant clinical hospital pharmacy experience is desirable.
• Must provide documentation of state licensure and CE as required by the state of KY.
• Maintain BLS & ACLS certifications.
• Advanced clinical training or PGY1 residency preferred.

Responsabilidades

• Provide drug preparation and distribution for medications; evaluate input and verification of medication orders.
• Serve as a drug information resource and advise providers and health care professionals.
• Monitor and manage medication regimens per protocol or consult.
• Participate in antimicrobial and opioid stewardship programs.
• Provide medication counseling to patients and assist with transitions to care.
• Collaborate with inter-professional teams on patientcare units; participate in unit-based huddles when available.
