Qualifications

Responsibilities

• Complete comprehensive assessment within 24 hours of admission.

• Provide psycho-social intervention for high-risk patients.
• Document timely and accurately in the Tenet Case Management system.
• Coordinate care with care team, patient, family, and post-acute providers.
