Children’s Hospital of Michigan is seeking a Social Worker to facilitate care along a continuum, coordinating resources to support optimal health and appropriate utilization of services. The role includes assessing transition needs, identifying risk for readmission, and implementing evidence-based psycho-social interventions to promote safe discharge and timely throughput.

Responsibilities span transition management, care coordination, education, and ensuring regulatory compliance.

Responsabilidades

• Completes comprehensive transition assessment within 24 hours of admission to identify the anticipated plan.
• Develops and implements transition plans with patient choice and available resources.
• Provides psycho-social assessment and intervention for high-risk readmission patients.
• Ensures transition plan elements are implemented and communicated to care team and post-acute providers.
• Delivers information to patients to support informed post-acute care decisions.
• Maintains timely, complete documentation in the Case Management system.
• Maintains accurate patient demographics and insurance information.
