Qualifications

Responsibilities

• Perform psychosocial assessments to identify patient health goals and coordinate services with the RN Case Manager for effective transitions.
• Meet with patient/family regularly to support discharge planning and continuity of care.
• Identify social/psychosocial needs and coordinate community resources as needed.
• Develop and revise care plans to improve transitions, LOS, and patient activation.
• Collaborate with physicians and the multidisciplinary team to achieve goals and mentor others.
