Job summary

Baptist Health in South Florida seeks a licensed clinical social worker to facilitate the safe movement of patients through the continuum of care and coordinate discharge planning for appropriate use of post-hospital services. MSW degree and LCSW licensure within 6 months are required.

The role requires crisis intervention skills, strong communication, and collaboration with medical staff to ensure patient-centered care and smooth transitions.

Qualifications

• Masters degree required.
• LCSW licensure required within 6 months of job entry date.
• Social Work Supervisors hired prior to 2001 may remain in their current role without licensure, however they must directly report to an LCSW.
• Qualified Supervisor status to supervise MSW interns for LCSW licensure preferred.
• Demonstrates expert knowledge in family centered social work for hospitalized patients with complex transition of care needs.
• Knowledge of crisis intervention, interdisciplinary collaboration, and case management principles, medical terminology, procedures and community resources.
• Ability to communicate effectively, maintain objectivity, deal sensitively with a diverse patient population and manage staff.
• Previous experience in discharge planning, knowledge of clinical and social factors affecting discharge, socioeconomic needs, patient and family centered care.

Responsibilities

• Facilitates the safe movement of patients through the continuum of care, while ensuring appropriate utilization of acute and post-hospital services.
• Represents department as member of a specialized team and in professional organizations.
• Facilitates group meetings and education.
• Acts as a department liaison with community social and health services.
• Participates in department activities and programs.
• Maintains accountability and performs administrative duties.
• Other tasks as needed.
