Qualifications

• Master’s degree in social work required.
• LMSW licensure required; LCSW preferred.
• Minimum two years of clinical social work experience in a medical/clinical setting.

Responsibilities

• Perform psychosocial assessments to identify health goals and coordinate services with the RN Case Manager.
• Identify social/psychosocial needs of patients and families.
• Develop and revise discharge plans to ensure continuity of care, reduce LOS, and support patient activation.
• Collaborate with physicians and the multidisciplinary team to meet discharge goals.
• Complete assigned goals and documentation.
