Job summary

Children's Hospital of Michigan seeks a Clinical Social Worker – Case Management to facilitate care coordination across transition planning and discharge processes. The role emphasizes psychosocial assessments, caregiver education, and effective communication with the healthcare team to optimize patient outcomes.

Responsibilities include complex assessments, care coordination, and timely documentation in the Tenet Case Management system, with focus on readmission prevention and safe post-acute

Qualifications

• Master’s degree in Social Work from a CSWE-accredited program.
• Current license as a Licensed Master’s Social Worker in Michigan, or a limited license with full licensure within 3 years.
• Two years of acute hospital experience preferred.
• Must complete and demonstrate competency in the Tenet Case Management documentation system within 30 days of hire.
• Accredited Case Manager (ACM) preferred.

Responsibilities

• Complete comprehensive psycho-social assessments and transition planning to identify discharge needs.
• Coordinate care to ensure timely throughput and prevent readmission.
• Educate physicians, patients, families and caregivers regarding plan of care and resources.
• Maintain accurate documentation in the Tenet Case Management system.
• Lead or participate in multidisciplinary case conferences as required.

Education

Master’s degree in Social Work (CSWE-accredited)

Accredited Case Manager (ACM) preferred
