Job summary

Phoenix Children’s is seeking a licensed Master Social Worker for a PRN Social Work role within Care Management. The position supports patients and families through bio-psychosocial assessments, treatment planning, and transition planning, working as part of an interdisciplinary team.

Responsibilities include conducting psychosocial assessments, coordinating care, and providing therapy as needed. Requires AZ LCSW/LSW licensure, CPR/First Aid, and strong communication skills to support discharge

Qualifications

MSW required for pediatric social work role.
Strong communication and collaboration with interdisciplinary teams.
Experience with family-centered care and crisis intervention preferred.

Responsibilities

Conduct psychosocial assessments and interventions for patients and families.
Identify therapeutic goals to improve family functioning and adherence to care plans.
Document clinical records and coordinate with care teams for discharge planning.
Develop and coordinate comprehensive treatment plans and interventions.
Participate in care conferences and interdisciplinary rounds as needed.
Conduct individual, group, and family therapy sessions when appropriate.
