Job summary

Oak Street Health seeks a Medical Scribe to support primary care providers by documenting patient encounters in real time and producing structured notes with history, assessment, plan and exam.

The role emphasizes accurate CPT/ICD-10 coding, after-visit summaries, and collaboration with the care team during daily huddles; strong communication and learning on-site are required. Commitment of 1 year+ preferred.

Qualifications

• Knowledge of medical terminology and common medications [required]
• Prior clinical experience, including shadowing and/or volunteering [strongly preferred]
• Prior scribe or transcription experience [preferred but not required]

Responsibilities

• Documenting patient encounters in a structured note with history, assessment, plan and physical exam
• Assigning CPT and ICD-10 codes
• Preparing After Visit Summaries
• Consulting with the provider to ensure accurate documentation
• Clinical Documentation Improvement: reviewing medical records and leveraging population health tools
• Daily Huddles and Clinical Documentation Reviews support
• Placing orders and referrals and assisting the care team with documentation tasks
