Job summary

Oak Street Health is seeking a Medical Scribe to assist primary care providers with real-time documentation. You will observe patient visits in the exam room, capture detailed histories, and help ensure accurate coding and documentation for each encounter.

Ideal candidates have strong medical terminology knowledge, clinical exposure, and typing speed of 70+ WPM. This full-time role supports remote? in-clinic teams and emphasizes HIPAA compliance and high-quality patient care.

Qualifications

• Knowledge of medical terminology and common medications.
• Clinical experience including shadowing or volunteering preferred.
• Scribe or transcription experience preferred but not required.

Responsibilities

• Document patient encounters in a structured note with history, assessment and plan.
• Join providers in exam rooms to observe visits and document care.
• Assign CPT and ICD-10 codes for encounters.
• Prepare After Visit Summaries and support documentation review.
• Coordinate with providers to ensure accurate clinical documentation.
• Support the care team and administrative tasks as needed.
