Job summary

Oak Street Health seeks a Medical Scribe to support primary care providers by documenting patient encounters in real time, assisting with coding and documentation, and preparing after-visit summaries. The role emphasizes accuracy, compliance with HIPAA, and teamwork within a value-based care setting.

Ideal candidates will have strong medical terminology knowledge, excellent listening and communication skills, and the ability to adapt to evolving clinical workflows.

Qualifications

• Knowledge of medical terminology and common medications (required).
• Prior clinical experience including shadowing/volunteering (strongly preferred).
• Prior scribe or transcription experience (preferred but not required).

Responsibilities

• Documenting patient encounters in structured notes (history, assessment, plan, exam).
• Assigning CPT and ICD-10 codes; preparing after-visit summaries.
• Clinical documentation improvement and reviewing medical records.
• Administrative support for the provider and care team; other duties as assigned.
