Job summary

Personal Touch Home Aides of New York seeks a highly organized Clinical Scheduler (Home Care) to coordinate clinician visits and maintain patient records in EMR. This full-time, in-person role is based out of Brooklyn, NY and requires strong knowledge of NYC geography, excellent communication, and meticulous data management.

The ideal candidate will manage multiple priorities, support audits, and ensure timely service delivery while maintaining top-quality patient care in home settings.

Qualifications

• Strong knowledge of NYC boroughs, neighborhoods and travel patterns.
• Proficient with Excel, Word, PDF editing and EMR systems.
• Accurate data entry and ability to maintain spreadsheets and trackers.

Responsibilities

• Coordinate schedules and track clinician visits (SOC, supervisory, preceptorship and follow-ups).
• Staff referrals and active patient coverage; maintain spreadsheets for patients and service areas.
• Cluster visits geographically using NYC borough knowledge and travel patterns.

Job Summary

Our Certified Home Health Agency (CHHA) is seeking a highly organized Scheduler and Clinical Support Specialist to support our clinical operations. This role is responsible for coordinating and scheduling field clinicians visits, maintaining accurate and up-to-date patient records in EMR and assisting the nursing department with audit preparation and compliance activities. The ideal candidate is detail-oriented, proactive, good communicator, and comfortable working in a fast-paced environment with multiple priorities and working knowledge of New York City geography—including boroughs, neighborhoods, and travel patterns. They must be confident using technology, running reports, managing spreadsheets.

Responsibilities

• Coordinate the schedules, confirm, and track all clinician visits, including SOC, supervisory, preceptorship and clinical follow-up visits.

• Responsible for staffing all new referrals and active patients maintaining all spreadsheets for active patients and service areas.

• Clustering visits geographically using strong knowledge of NYC boroughs, neighborhoods, and travel patterns

• Communicate with field clinicians and managers to ensure smooth workflow and timely service delivery.

• Serve as administrative support to the clinical department, including organizing charts, uploading documents, and managing deadlines.

• Assist with internal and external audits (DOH, OMIG, MLTC), including gathering documentation, compiling visit logs, and ensuring data accuracy.

• Monitor email queues and task lists to ensure timely responses to inquiries and requests.

• Provide excellent customer service when interacting with patients, families and clinicians.

• Perform other duties as assigned to support clinical operations.

Desired Skills and Experiences

• Strong working knowledge of NYC boroughs, neighborhoods, and surrounding areas to efficiently cluster and route clinician visits.

• Strong computer proficiency (Excel, Word, PDF editing, EMR).

• Ability to manage spreadsheets, create simple trackers, and handle data entry with high accuracy.

• Fast and accurate typing skills.

• Ability to scan, upload, and organize documents logically and efficiently.

• Excellent communication skills and professional phone/email etiquette.

• Strong attention to detail and ability to manage multiple priorities.

• Experience in home care, CHHA (preferred) or LHCSA.
