Job summary

Personal Touch Home Care in Brooklyn, NY is seeking a detail‑oriented Clinical Scheduler to coordinate field clinician visits and manage patient records in our EMR system. This full‑time, in‑person role requires strong knowledge of NYC boroughs and travel patterns, plus excellent communication and data‑entry skills.

You will support the nursing department, prepare audits, and help ensure timely, high‑quality home care for our patients in a fast-paced environment.

Qualifications

• Detail-oriented and proactive with strong communication skills.
• Comfortable using technology, running reports, and managing spreadsheets.
• Experience in home care or CHHA/LHCSA is preferred.

Responsibilities

• Coordinate schedules, confirm and track clinician visits (SOC, supervisory, follow-up).
• Staff new referrals and active patients; maintain spreadsheets for service areas.
• Cluster visits geographically using knowledge of NYC boroughs and travel patterns.
