Job summary

Hartford HealthCare at Home is seeking a QAPI Specialist to lead clinical quality improvement initiatives across the agency. You will analyze data, review records, and develop action plans to improve patient outcomes and safety. This role requires strong communication and the ability to collaborate with diverse teams.

The position emphasizes regulatory compliance, data integrity, and the ability to travel to branches as needed to support performance improvement efforts.

Qualifications

• Associate of Science Degree is required.
• 3 years of certified home health experience.
• Value Based Purchasing, PDGM, CAHPS preferred.
• CT licensure in field of specialty.
• Experience in providing data analysis and statistical processes in quality reporting.
• Ability to design and develop performance measures.
• Experience in providing education.
• Experience in facilitation of PI teams using quality improvement methodology.
• Excellent written and verbal communication skills.

Responsibilities

• Lead development of Performance Improvement Plans based on data.
• Conduct clinical record audits and provide educational feedback.
• Compile trends and reports on quality indicators and outcomes.
• Coordinate with staff to implement evidence-based guidelines.
• Ensure compliance with federal and state regulations and ICD-10 coding.
