Qualifications

• Bachelor/Master/Doctorate in Physical Therapy from an accredited program.
• Current license from the state Board of Physical Therapy Examiners.
• Experience with SNF population or similar settings preferred.

Responsibilities

• Provides direct patient care and evaluates functional needs to plan therapy.
• Designs and implements individualized plans based on assessments and family involvement.
• Performs therapy within defined standards of care and facility policies.
• Directs support staff to achieve patient goals.
• Communicates effectively with patients, families, and staff to improve outcomes.
• Maintains documentation of evaluations, plans, and progress as required.
• Keeps up to date with professional literature and participates in educational activities.
• Ensures safety, infection control, and quality improvement practices.
