Job summary

White Oak Healthcare is seeking a licensed Physical Therapist to develop and implement individualized therapy programs for patients in a skilled nursing facility. The role focuses on assessment, treatment planning, and collaboration with a multidisciplinary team to maximize functional outcomes.

The PT will maintain documentation, adhere to standards, and participate in ongoing professional development and education while working with patients, families, and staff to support optimal care.

Qualifications

• Graduate degree in Physical Therapy (Bachelors, Masters, or Doctorate).
• Licensed to practice physical therapy in the state.
• Compliant with state, federal and professional standards.

Responsibilities

• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
• Designs and implements a plan of care for patients based on a thorough assessment and includes significant others when appropriate.
• Conducts therapy practice within defined standards of care.
• Directs the assistance of support staff to achieve patients’ functional goals.
• Communicates effectively with other professionals and support staff to achieve positive patient outcomes.
• Follows facility and departmental policies, quality improvement, safety, and infection control standards.
• Documents evaluations, treatment goals, and plans; regularly updates documentation and prescriptions as needed.
• Engages in ongoing professional development and education.
