Job summary

Belleville Healthcare and Rehabilitation Center is seeking a Physical Therapist to develop individualized therapy programs, assess patients, and implement treatment plans to maximize progress toward functional goals.

You will collaborate with other health professionals, document evaluations and goals, and participate in ongoing professional development while maintaining quality patient care in a SNF setting.

Qualifications

• Graduate of an accredited Physical Therapy program (Bachelors/Masters/Doctorate).
• Current license from the state Board of Physical Therapy Examiners.
• Proficiency with applicable state, federal, and professional standards.

Responsibilities

• Provides direct patient care and evaluates functional needs and outcomes.
• Designs and implements a plan of care based on assessment.
• Maintains documentation of evaluations, goals, and plans.
• Communicates with professionals and staff to achieve positive outcomes.
• Ensures adherence to policies, safety, and infection control.
• Participates in ongoing education and professional development.
