Job summary

Qualifications

• Bachelor's degree in Physical Therapy
• Physical Therapist license from a participating state in the National Licensure Compact
• Basic Life Support (BLS) from the American Heart Association

Responsibilities

• Evaluates patients admitted to physical therapy and conducts initial evaluation per department protocol
• Develops evidence-based treatment plans with measurable goals and timelines
• Implements therapy activities to meet patient and family goals per care plan
• Documents evaluations, progress notes, and treatment records per practice standards
• Reassesses plans and adjusts as function changes and discharges appropriately
• Communicates with patients, families, and health care team while maintaining confidentiality
