Job summary

CommonSpirit Health at Home is seeking a compassionate Physical Therapist to provide in-home therapy, improve functional movement, and support rehabilitation goals. You will collaborate with an interdisciplinary team to develop individualized care plans and promote recovery in the patient’s home environment.

Responsibilities include conducting evaluations, delivering therapy, educating families, tracking progress, and ensuring compliant documentation. A licensed PT with CPR/BLS is required.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Active PT license in the state of practice.
• CPR/BLS certification current and maintained.

Responsibilities

• Conduct evaluations of patients' mobility and functional status.
• Implement exercises, manual therapy, and modalities to improve function.
• Educate patients and families on exercises, safety and recovery.
• Monitor progress and adjust plans to optimize outcomes.
• Maintain accurate patient records in compliance with laws and policies.
