• Bachelor's degree in Physical Therapy is required.
• Licensed Physical Therapist in state of employment or NLC participation.
• BLS certification from the American Heart Association.

Responsibilities

• Evaluates patients referred to physical therapy and starts timely evaluations.
• Develops treatment plans with measurable goals and timelines.
• Selects and applies therapy activities to meet goals per care plan.
• Documents evaluations, progress notes, and treatment records per standards.
• Reassesses plans and discusses dispositions with the care team.
• Communicates with patients, families, and staff while protecting privacy.
