Qualifications

• Pharmacy Residency (PGY1) or 4 years of demonstrated clinical experience.
• Licensed or eligible for licensure as a Registered Pharmacist in PA or eligible to obtain licensure within 120 days.

Responsibilities

• Provides pharmaceutical care in specialty area, reviews patient history, and offers drug therapy recommendations.
• Plans, implements, measures clinical processes to improve outcomes and minimize adverse events.
• Coaches and trains pharmacy staff, residents, students and other healthcare providers.
• Promotes interdisciplinary teamwork and implements clinical pathways using best-practice evidence.
• Participates in planning pharmaceutical care to meet department goals; may assist Pharmacy Practice Supervisor.
• Mentors Pharmacy Residents to develop clinical specialization.
• Contributes to research/publishing and presents findings at meetings.
• Performs other duties as assigned.
