Job summary

Qualifications

• Minimum Pharmacy Residency (PGY1) or 4 years of demonstrated clinical experience.
• Licensed or eligible for licensure as a Registered Pharmacist in PA; eligible to obtain licensure within 120 days.

Responsibilities

• Provides pharmaceutical care in their specialty practice area including review of patients' drug/medical history and drug therapy recommendations.
• Plans, implements and measures clinical processes to improve outcomes and reduce adverse drug events.
• Coordinates Drug Utilization Evaluations (DUEs) and analyzes data for action by committees.
• Creates a culture of learning and coaching of staff, residents, students and healthcare providers.
