Allegheny Health Network is seeking a pharmacist to provide pharmaceutical care within a specialty practice, review patients' drug histories, and offer drug therapy recommendations to optimize outcomes.

The role emphasizes leadership, staff development, and collaboration across clinical teams to improve safety and effectiveness of medication use, while ensuring compliance with HIPAA and organizational policies.

Qualifications

• Pharmacy Residency (PGY1) or 4 years of demonstrated clinical experience.
• Licensed or eligible for licensure as a Registered Pharmacist in the state of PA or eligible to obtain licensure within 120 days.
• PGY2-Residency (preferred)

Responsibilities

• Provides pharmaceutical care in their specialty practice area including reviewing and evaluating patients' drug/medical history, providing drug therapy recommendations, and patient counseling.
• Plans and measures clinical processes to improve outcomes while minimizing adverse events; coordinates DUEs and analyzes data for action by committees.
• Creates a culture of learning and supports development of clinical staff through coaching and training of Pharmacists, Pharmacy Residents, students and healthcare providers.
• Promotes interdisciplinary teamwork and development of clinical pathways to improve medication safety and utilization.
• Provides input in planning pharmaceutical care to meet department goals and may assist Pharmacy Practice Supervisor/Coordinator.
• Instructs, mentors and empowers Pharmacy Residents to become clinical specialists where applicable.
• Contributes to the medical community by presenting research or publishing results.
