Job summary

Qualifications

• Licensed pharmacist in PA or eligible within 120 days.
• Pharmacy residency or 2+ years of clinical hospital experience.
• Strong knowledge of drug therapy monitoring and formulary management.

Responsibilities

• Performs pharmacist duties and supports drug therapy monitoring.
• Conducts drug formulary reviews and generates evaluation reports.
• Educates Pharmacy staff, Residents, nurses and medical staff.
• Participates in interdisciplinary rounds and committee work.
• Develops and coordinates pharmaceutical programs and initiatives.
• Ensures adherence to HIPAA and privacy policies.
