Job summary

Highmark Health is seeking a pharmacist to optimize patient care through clinical services, drug therapy monitoring, and formulary reviews. You will educate staff, residents, students, and the healthcare team while conducting pharmacotherapy interventions to reduce adverse events.

You will lead clinical processes, coordinate drug utilization reviews, and help advance standard care pathways with a focus on safety, efficacy, and cost-effectiveness.

Qualifications

• Pharmacy Residency (PGY1) or 4 years of demonstrated clinical experience.
• Licensed or eligible for licensure as a Registered Pharmacist in the state of PA or eligible to obtain licensure within 120 days.
• PGY2-Residency preferred.

Responsibilities

• Provides pharmaceutical care in their specialty practice area including reviewing and evaluating patients' drug/medical history and providing drug therapy recommendations.
• Plans, implements and measures clinical processes to improve treatment outcomes while minimizing adverse drug events.
• Creates a culture of learning and sharing to develop clinical staff through coaching and training of pharmacists, residents and students.
• Promotes interdisciplinary teamwork through development and implementation of clinical pathways and protocols for medication safety and utilization.
• Provides input in planning pharmaceutical care to meet department goals; may assist as Pharmacy Practice Supervisor/Coordinator.
• Instructs, mentors, and empowers Pharmacy Residents to become clinical specialists where applicable.
• Contributes to the medical community by presenting research or publishing results.
